What is NHTD, and how can we help?

Nursing Home Transition/Diversion Waiver (NHTD) is a Medicaid Waiver program that empowers individuals to live independently within their communities rather than residing in nursing homes or other institutional settings. It focuses on providing person-centered planning, support services, and resources to help individuals regain their independence, enhance their quality of life, and integrate into their communities.

Independent Living Skill Training

The trainer empowers individuals through instruction and support, fostering self-sufficiency and confidence for improved daily living and quality of life.

Adult Structured Day Program

Adult Structured Day Programs plays a vital role in supporting adults with disabilities to lead fulfilling and meaningful lives, with community Intergration and cognitive development through productive activity

Service Coordination

Comprehensive Care Planning: care plans that address various aspects of their well-being, including health, housing, employment, education, and social integration.

Eligibility Requirements for NHTD Program

Be capable of living in the community with needed assistance from available informal supports, non-Medicaid supports and/or Medicaid State Plan services and be in need of one or more waiver service 64 years with a physical disability or chronic illness

Be eligible for nursing home level of care

Be authorized to receive Medicaid Community
Based Long Term Care

Be at least 18 years of age or older

Be considered part of an aggregate group that can be cared for at less cost in the community than a similar group in a nursing home.

Choose to live in the community as a participant in this waiver rather than in a nursing home

Not participate in another HCBS waiver
